My signature at the bottom of this form attests to the fact that I have read, understood, and agreed to all statements included in this document.

1. In order to earn three (3) hours of course credit for Sociology 488, I shall work a minimum of 120 hours at the agency with which I am affiliated.

2. I shall keep a daily journal of my field observations and make it available for discussion with the Coordinator of Interns. I understand that this journal will become part of my permanent Department record.

3. I shall write a research paper based on my field experience, in consultation with the Internship Program Coordinator.

4. I shall attend and complete all preparations, training sessions, conferences, etc. required by my agency supervisor.

5. I shall abide by my agency’s rules and expectations for interns.

6. I understand that my final grade for this Internship will be based on attendance at all class meetings scheduled by the Internship Program Coordinator, the quality of my journal and my research paper, and an evaluation by my field supervisor.

7. I understand that should my Internship be terminated by my field supervisor for inappropriate conduct while on the job, I shall receive the grade of E for the course.
