My signature at the bottom of this form attests to the fact that I have read, understood, and agreed to all statements included in this document.

1. In order to earn three (3) hours of course credit for

Sociology 488 I shall work a minimum of 75 hours at the agency with which I am affiliated.

2. I shall keep a journal of my observations in the field make it available for examination and discussion with the Coordinator of Sociology Interns.

3. I shall write required research papers based on my observation and analysis of my internship field experience. I understand that the specific topic and form of these papers will be presented in class.

4. I shall attend and complete all preparations, training

sessions, conferences and meetings required by my agency supervisor.

5. I shall abide by my agency's rules and expectations for interns.

6. I understand that my final grade for the Practicum will be based on my daily journal, the research papers I write, class tests and assignments, and an evaluation completed by my field supervisor.

7. I understand that my attendance is required at all classes scheduled for Practicum Interns by the Coordinator of Sociology Interns.

8. I acknowledge the fact that if my internship is

terminated by my field supervisor for inappropriate conduct while on the job, I shall receive the grade of E for that internship.
